The plasma clearance of topiramate does not change in elderly patients without kidney disease.
In patients receiving concomitant therapy with antiepileptic drugs, which induce enzymes involved in drug metabolism, the metabolism of topiramate was increased by 50%.
Topiramate 25mg is effectively excreted by hemodialysis. Prolonged hemodialysis may cause the blood concentration of topiramate to fall below the amount required to maintain anticonvulsant activity.
An additional dose of Topamax may need to be administered to avoid a rapid drop in plasma concentrations of topiramate during hemodialysis.

Caution should be used in renal or hepatic insufficiency, nephrourolithiasis (including past or family history), hypercalciuria.
